I agree with Howie that this requirement should be added to the main resolution. As to the fillable PDF form, my point is that it should include a note or statement about this requirement so that hybridizers will know up front what the rules are. If this requires making the form 2 pages long, then we should give it some serious consideration. I imagine someone making 100 copies of the form from the pdf document, and using them for years, and forgetting all about the requirements. If the reminder is on the form, then there is no excuse for not knowing.BTW there is a typo in the italicized statement in Neil's message.
